Assessment Tasks

Copy and paste from the following data to produce each assessment task. Write these in plain English and spell out how, when and where the task is to be carried out, under what conditions, and what resources are needed. Include guidelines about how well the candidate has to perform a task for it to be judged satisfactory.
Copy and paste from the following performance criteria to create an observation checklist for each task. When you have finished writing your assessment tool every one of these must have been addressed, preferably several times in a variety of contexts. To ensure this occurs download the assessment matrix for the unit; enter each assessment task as a column header and place check marks against each performance criteria that task addresses.

Obtain written or verbal delegation for an allied health activity from the Dietitian. 
Obtain information from relevant sources and delegating Dietitian, according to organisational policy and procedures. 
Discuss and confirm with delegating Dietitian educational needs, process, timing, monitoring feedback and work health and safety (WHS) requirements. 
Confirm details regarding the person's indicators that may need to be considered with the delegating Dietitian. 
Prepare educational materials and products as delegated by the Dietitian. 
Develop or modify resources as delegated by the Dietitian. 
Confirm materials and products are suitable for the person’s setting with the delegated Dietitian. 
Determine the person's availability. 
Confirm the purpose of the information or education, based on the individualised plan with the person. 
Support the person to make meal choices that are consistent with the individualised plan as delegated by the Dietitian. 
Provide practical dietary education to support meal and food choices consistent with individualised plan. 
Use nutrition and dietetic resources and equipment. 
Seek feedback on the person’s understanding. 
Monitor the person according to individualised plan. 
Identify the person’s deviations from the individualised plan and report to delegating Dietitian. 
Identify and report indicators requiring dietary intervention to the delegating Dietitian in line with organisational procedures. 
Seek the person’s feedback on satisfaction with the dietetic service. 
Provide assistance as delegated by the Dietitian. 
Provide feedback to the delegating Dietitian regarding the person’s progress. 
Seek assistance when the person presents with needs or signs outside limits of scope of role. 
Report the person’s difficulties and concerns to the delegating Dietitian. 
Implement variations to the education program and material as delegated by the Dietitian. 
Clean equipment according to manufacturer requirements. 
Store educational materials and equipment according to manufacturer requirements and organisational procedures. 
Report equipment faults in accordance with organisational procedures. 
Report shortages of educational material in accordance with organisational procedures. 
Document and report person's information in line with organisational procedures. 
Document the person’s response to information or education in line with organisational procedures. 
Maintain the person's records, according to organisational procedures.
